Do you know of any reason why you should not take part in physical activity? eg; high blood pressure, asthma, diabetes, previous injuries, chest pains, joint or bone issues, diziness, dyspraxia, are you a smoker, pregnancy, etc.
PERSONAL INJURY WAIVER AND PHOTOGRAPHY / MEDIA CONSENT 

I understand that my participation in exercise programs, classes or events and any related activity conducted by Peter Sneddon is voluntary and at my own risk.

 

I am aware that exercise can be physically stressful and, in certain instances, even result in injury or cause of death. The levels of exercise that I will perform will be at my own pace, based upon my cardiorespiratory fitness, muscular strength, movement capabilities and endurance levels.

 

I hear by state that I will inform Peter of any symptoms (E.G. fatigue, shortness of breath, chest discomfort, any pain/discomfort/concern for my safety) during my participation in exercise. If I have high blood pressure, diabetes, a heart condition, any existing injury, recent surgery or if I am taking any prescribed medications that could affect my performance, I will inform Peter prior to participating in any exercise.

 

 

I understand that I will be given instructions on how to perform an exercise and use equipment and I will ask the trainer if there is anything I do not understand before starting to exercise. 


 

Peter Sneddon, will not be liable for any injuries or damage arising from participation in training to the undersigned. It is agreed that Peter shall not be responsible or liable to the undersigned for articles lost or stolen whilst training with him.

 

By taking part in training with Peter Sneddon (including but not limited to personal training, group training or training events) I agree that full rights to the use of images resulting from photography/video filming during my training, and any reproductions or adaptations of the images can be used for fundraising, publicity or other purposes to help achieve the aims and objectives of Peter Sneddon Personal Training. 

This might include (but is not limited to), the right to use them in printed and online publicity, social media, press releases and funding applications.

 

Permission will be asked by Peter Sneddon before taking photos in 1-1 or 2-1 settings too.

 

I consent to Peter Sneddon photographing and/or filming me and for these images or videos to be used for marketing and other promotional uses, including (but not limited to) it’s use on www.petersneddonPT.com and its associated social media sites. 
 

CLIENT - TRAINER AGREEMENT

An initial consultation will determine your fitness requirements and help you to set appropriate goals. Your personal aims and lifestyle information will then be used to design an appropriate training program.

 

The following when signed will constitute the full agreement between Peter Sneddon and client.

 

Peter Sneddon will act as a Personal Trainer in the development of an exercise programme during the agreed sessions.

 

In the consideration of the service, the client shall pay in advance according to the session rates charged by the trainer. No refunds will be given, however extra sessions can be given to a friend or family member or made up in other months. 


 

I agree to the following conditions which I will follow to the best of my ability.

 

  • You will be required to have an initial assessment with Peter unless an assessment is deemed unnecessary by Peter.

  • All appointments must be booked using Calendly. At least the day before a session.

  • Suitable workout gear and footwear must be worn.

  • Please inform Peter of any medical conditions you may have e.g. chest pains - before attendance, or any medication you may be taking. If unsure, please ask.

  • You will be required to give one full calendar month's notice of any cancellation or amendments to your payment plan(s) in relation to Peter Sneddon.

  • Peter is only required to wait for 15 minutes past the agreed start time. After this the client will forfeit the amount for the session. If the client arrives within the 15 minutes, the trainer will only complete the time left of the original session. No extra time will be given.
